Fee Agreement …
Fees are due when our candidate is 1) hired by your company or 2) hired at any time within two years after your
contact with our candidate.
Candidates are referred to you in confidence. Should you refer or otherwise identify our candidate to another
company within the two-year time limit and that company in turn hires our candidate, responsibility for the
fee payment will be shared by you, the referring company, and the hiring company.
Susan Lee & Associates will verify references on each candidate to maximize assurance of the required skills
and experience for the client's job profile. Information obtained by us in this manner is kept confidential.
You are responsible for conducting your own qualification and reference verification. In further consideration
of our services, you agree to verify employment eligibility of the referred candidates you hire. You agree to
mail us a photocopy of the completed I-9 Immigration Form required by Federal law immediately
upon its completion.
It is our policy not to discriminate in the acceptance or referral of applicants on account of race, color,
religion, sexual orientation, national origin, marital status, age, or disability.
Fees for a permanent position of employment are computed at a percentage of the Total Estimated Gross Annual
Income of the candidate's first year of employment, which includes a reasonable estimate of the commissions,
bonuses, incentives and other forms of taxable income that will be earned within the first year. All income
estimates will be mutually agreed upon by the candidate, Susan Lee & Associates, and your firm prior to
extending any offer of employment to our candidate.
Contingency Fee Schedule …
1% per $1,000 Income
Up to a Maximum of 30%
($30,000 Annual Income and above = 30% fee)
Fees are invoiced when our candidate begins employment. Payment is due upon receipt for our guarantee
to be valid. If the amount due is not paid within five days of receipt of invoice, a 1.5% per month
late charge will be added to the unpaid balance. If Susan Lee & Associates incurs a cost for collecting
your unpaid balance, including, but not limited to, reasonable attorney's fees, you agree to pay these
costs. The jurisdiction of the courts of the State of Minnesota and its laws shall govern this agreement.

Guarantee …
A candidate terminated from employment for any reason other than lack of work within the first 30 days
of employment with you will be replaced without a fee charge, provided that:
You notify Susan Lee & Associates in writing within five business days as to the facts relating to
the termination
and
You have paid all fees for placement of the terminated candidate due Susan Lee & Associates within
5 days of receipt of invoice
Susan Lee & Associates will work with you to replace a terminated candidate for up to 6 months.
In the event that Susan Lee & Associates can not find a suitable replacement after 6 months of
searching, a reasonable settlement will be offered.
